Community Connectors

The Healthy Mind Community Connectors service offers practical and emotional support to individuals whose wellbeing may be impacted by physical health, social circumstances, or life challenges. It is designed to complement clinical care by addressing non-medical factors that influence mental health and wellbeing.

Who is it for?

This service is suitable for adults who may benefit from:

· Increased social connection

· Access to housing, benefits, or debt support

· Access to employment, volunteering, or training

· Peer support and community engagement

· Help navigating local services and resources

It is particularly helpful for individuals experiencing low mood, anxiety, isolation, or stress related to social or practical issues.

What we offer:

It all begins with a conversation about what matters to the individual and what could help them to stay well. We are available by phone, video call or in person.

Together, the individual and Community Connector create a plan with clear next steps. We’ll provide information, connect individuals to local services, and check in to see how things are going.

We work at the individual’s pace, whether that’s a one-off chat or support for up to 6 weeks.

We use the Five Ways to Wellbeing to help individuals build a balanced and fulfilling life:

· Connect – fostering relationships and community ties

· Be Active – encouraging movement and physical activity

· Keep Learning – promoting personal growth and skill-building

· Take Notice – supporting mindfulness and self-awareness

· Give – enabling contribution and purpose

Eligibility Criteria for Referral

· Adults aged 18 and over

· Living within the South East and Central Essex boundary

· Registered with a GP in South East and Central Essex

· Experiencing low to moderate mental health needs or social challenges

· Seeking support with wellbeing, social connection, or practical issues

This service is not intended for individuals with high acuity mental health needs requiring secondary mental health services.

To receive this support a referral is required. Referrals can be made directly by physical and mental health professionals including the following: GP, Primary and Secondary Care.
